Test to Release Booking Form

Important Information

• Complete the form below to book your test – please provide sufficient notice so that we can send the testing kits to your nominated address in time for you to take your sample on the 5^th full day after your arrival in the UK.
• Orders must be received by 3.00pm for a kit to be despatched that day.  Cellmark does not send out kits on a Saturday or Sunday.  We cannot accept responsibility for any delays during postage.
• You must not take your sample earlier than the 5^th full day of self-isolation or it will result in an invalid test and you will not be able to exit self-isolation early.
• After completing the booking form you will be given a booking reference that you must enter onto your Passenger Location form.
• You will then be directed to pay for your Test to Release test at £95 including postage and VAT.
