Important Information

  • You must activate your kit by completing the form below before returning your samples to prevent any delays in getting your result.
  • You'll see that we need you to provide your booking reference. This is the reference you received when buying your kit(s) and have entered on your passenger locator form. This reference can also be found on the delivery note included in the kit.

 


(Must be on/after 5th full day after arrival in the UK)

  • I consent to Cellmark carrying out a COVID-19 test on the donor’s sample (if the donor is a child I confirm that I have the legal authority to give consent).
  • I consent to the donor’s personal details being stored and processed by Cellmark (please see Cellmark’s privacy policy for data retention timeframes).
  • I consent to the donor’s test result and personal details being reported to Public Health England, in compliance with public health legislation.
